Cost checklist · The full bill
How much will the complete plan cost?
A low opening price can leave questions about the next bill. Ask for the usual cost and the full payment terms.
How do I check the full cost?
Get an itemised written quote: visit fee, medicine charge, testing costs, delivery charge and follow-up costs.
For any membership, ask what the fee covers, when renewal happens and which steps end it.
A quoted medicine price and a total care bill answer different questions. Compare the same items in each offer.
Can you give me a price example?
The following example concerns a medicine’s price; it is not a Cleveland care quote.
Ozempic contains semaglutide, a medicine used by adults with type 2 diabetes to lower blood sugar.
On September 26, 2026, a new-patient cash offer for certain starting strengths of Ozempic injection was $199 for each month’s supply.
That price covered only the first two months’ supplies for adults living in the United States who met the offer’s conditions.
Later supplies were priced at $349 or $499 per month, with the price set by the strength.
The offer’s recorded end date was December 31, 2026.
These are medicine-only cash figures; they do not establish the cost of a visit, tests or follow-up.
What belongs on my payment checklist?
What can I ask my insurer?
Give your insurance plan the exact name of the proposed medicine, along with the reason it would be prescribed. Ask what you would owe for that use.
If the plan asks for prior authorization, that means it wants to agree to cover the medicine before the prescription is filled.
Keep the response with the quote. Ask the service what happens if the plan says no.
Does Ohio limit online visit fees?
As of September 28, 2026, Ohio forbids clinicians from adding certain separate charges to an online visit.
These include a “facility fee”, for the building, and an “origination fee”, for starting the visit.
The clinician also cannot add a fee for the equipment they use.
This rule does not limit what the clinician charges for their work during the visit or what the medicine costs.
Ask for the meaning of any fee whose name is unclear.

Where the price example comes from
These were the published cash prices on September 26, 2026; the offer may have changed since then.
Ask for the price and offer conditions that apply to you now.
Money paid under that cash offer does not reduce your insurance deductible—the amount you must pay before certain benefits of the plan begin.
The Ohio fee rule is recorded as of September 28, 2026.
